It says the site is "example.com" and all the nodes are "Example A", "Example B" and "Example C". When I change my tor circuit, I'm not sure if it's working.
I'm on Windows 7 Ultimate 64-bits.

This probably indicates an error in torbutton. Could you open the browser console (Tools > Web Developer > Browser console) and paste any errors you see after starting Tor Browser and opening a single website? Thanks.
Have same problem. (Tor 6.0.1 (based on Mozilla Firefox 45.2.0))
getFirstPartyURI failed for chrome://browser/content/browser.xul: 0x80070057
NS_ERROR_XPC_GS_RETURNED_FAILURE: Component returned failure code: 0x80570016 (NS_ERROR_XPC_GS_RETURNED_FAILURE) [nsIJSCID.getService] torbutton.js:1619:0
getFirstPartyURI failed for chrome://browser/content/browser.xul: 0x80070057
Is that reproducible? If so, how? Do you use a vanilla Tor Browser in the default setup?
Sorry for late reply but I don't use Tor often.
I'm using the Tor browser package with few modifications. Have a few addons like NoScript, Adblock and Better Privacy. Other then that it's using the default settings... as far as I can remember. Still have it and it's always "on". That is that I can't do anything about it so all I have to do to reproduce it is to open the browser again. Will test a few settings now and see if I can find anything useful.
Edit
Just disabled all addons except for TorLauncher and TorButton and that did not help.
