Since downloading and using the latest version of TBB, I have watched Tor unmask itself without notification, or justification on at least two separate occasions. I was able to determine this because I initially and at random times hit the home page button which takes me back to your IP check page and it said I was not configured to use TOR and my IP address was revealed. I thought it was an isolated issue, nonetheless, I have been frequently checking the home page for IP verification since that time and it happened again a few days later.
Each time it said that I was not using TOR in the middle of a Tor session, when it initially said CONGRATULATIONS YOUR BROWSER IS CONFIGURED TO USE TOR. Internet didnt get disconnected and Tor message log did not indicate any problems. At this point I may go back to the previous version of TOR just to be safe. It would appear this version is either bugged or some other issue exist.

Can you make sure it really is your own IP that is shown when this happens.
Also, please post which TBB version you are using and on which operating system.

Sometimes Tor Check believes you are not using Tor even if you are, due to exit node exiting from other IP than the one Tor Check knows about.

Needs information? OK. It seems to be that Tor "requires" FAQ for online security check services ;) , because:
behavior, described by the user, was seen on when it "detected" that user had 2 different IP addresses simultaneously: usually one "strange" and one from the same subnet as exitnode.
Also there is another situation: service doesn't recognize IP as Tor exitnode and gives not optimal recommendations.
